But for an elderly, catheterized patient, no growth" could mean many things: • Recent antibiotics suppressing bacterial growth • Fastidious organisms not growing on routine media • Non-infectious causes of fever: drug reactions, dehydration, or a blocked catheter I asked my technician to tell the son I'd speak with the doctor. Gram stain: No bacteria, many pus cells Culture: No growth after 48 hours Patient: Still febrile, counts high This can happen when prior antibiotics suppress bacterial growth, when organisms are too fastidious for routine media, or when the cause is non-infectious. In such cases, the microbiology report is not a verdict, it is a clue that must be integrated with clinical findings."
